string(7812) "<b><u>AGREEMENT</u></b><br>I understand that enrollment in Jewish
Community Center of Greater Orlando, Inc (JCC) programs is for the semester and that no
deductions are to be made or credits allowed for absences or voluntary/forced
withdrawal. All fees are non-refundable and non-transferable.
JCC Membership is provided complementary
with preschool tuition. Part-time preschool families membership ends May 31,
2019. Non-payment of fees may result in termination of services.
<br><span><i><br><span><b>FOR YOUTH PROGRAMS</b>:</span></i> A completed JCC
Youth Health Form must be submitted for each child before attendance is
permitted. By signing this agreement,
you are agreeing to the policies set forth in the parent handbook. All
participants must have a State of Florida School Entry Health Exam form and a
Florida Certification of Immunization form on file at their school.</span>
<br><span><i><br><span><b>FOR PRESCHOOL PROGRAMS</b>:</span></i> Prior to the start of school, you must provide copies of your
child?s State of Florida School Entry Health Exam form and Florida
Certification of Immunization form?both of these can only be obtained from your
pediatrician. The vaccination of all children enrolled in The Roth Family JCC?s
early childhood programs is essential in order to maintain a safe environment
and decrease the risk of transmission of preventable illnesses. Therefore, all
children enrolled in the Richard S. Adler Early Childhood Learning Center or
Shayna?s Village programs must be adequately immunized against the preventable
diseases as recommended by the American Academy of Pediatrics.</span>
You must also submit to the preschool office a patient?s
authorization for medical treatment/pickup authorization card, student survey,
influenza virus form, signed consent for observation and signed parent handbook
form. In addition, all parents are required to read the DCF booklet Know Your
Daycare Center.
<br><br>I understand the Program Director reserves
the right to cancel enrollment, dismiss a student or parent/guardian whose
mental condition, conduct, influence, or behavior is deemed unsatisfactory to
the best interest of the JCC and its students.
<br><br>I understand that the programs end at
designated times throughout the day and will make alternate pickup arrangements
should I be late. If I am more than 5 minutes late, I?ll be charged a $10 fee,
plus an additional $1 per minute I am late (for example, $11 for 6 minutes; $15
for 10 minutes).
<br><br>I give my children permission to
participate in JCC activities. I understand that even when every reasonable
precaution is taken, accidents can sometimes happen. Therefore, in exchange for
the JCC allowing me to participate in JCC activities, I understand and
expressly acknowledge that I release the JCC and its staff members from all
liability for any injury, loss, or damage connected in any way whatsoever to my
(or my children?s) participation in JCC activities, whether on or off JCC?s
premises. I authorize JCC staff to act on their best judgment in the event of
an emergency; including transporting myself or my child(ren) to the hospital
and authorizing treatment by a physician.
<br><br>My child is in good health and physical
condition, and I permit my child to participate fully in all program.
<br><br>I understand the JCC does not offer
medical insurance relating to all accidents and/or injuries that may result
from my or my family?s participation in JCC activities. I will be responsible
for maintaining any appropriate medical insurance for myself or my children.
<br><br>I give my permission to the JCC to use
without limitation and obligation, photographs, film footage, or video
recordings which may include my (or my family?s) image or voice for purposes of
promoting or interpreting JCC programs through advertising, social media or
other means.
<br><span><br>I have read and am voluntarily signing
this authorization and release. I understand that the JCC is not responsible
for personal property lost or stolen while members and/or program participants
are using JCC facilities or are on JCC premises, including all lost or stolen
property caused by the negligence of the JCC, its employees, independent
contractors, directors, members and guests.<br></span>
<span><b><br><br><u>WAIVER, RELEASE, AND INDEMNIFICATION AGREEMENT</u></b><b></b></span><span><u>

</span><br>In consideration for being permitted to
utilize the facilities, services, and programs of the Jewish Community Center
of Greater Orlando (JCC) for any purpose, including but not limited to
observation or use of facilities or equipment, or participation in any program
affiliated with the JCC, without respect to location, the undersigned, for
himself or herself and any personal representatives, heirs, and next of kin,
hereby acknowledges, agrees and represents that he or she has, or immediately
upon entering or participating will inspect and carefully consider such
premises and facilities or the affiliated program. It is further warranted that such entry into
the JCC for observation or use of any facilities or equipment or participation
in such affiliated program constitutes an acknowledgement that such premises
and all facilities and equipment thereon and such affiliated programs have been
inspected and carefully considered and that the undersigned finds and accepts
same as being safe and reasonably suited for the purpose of such observation,
use, or participation.
<br><br>IN FURTHER CONSIDERATION OF BEING PERMITTED
TO ENTER THE JCC FOR ANY PURPOSE, INCLUDING BUT NOT LIMITED TO OBSERVATION OR
USE OF FACILITIES OR EQUIPMENT, OR PARTICIPATION IN ANY PROGRAM AFFILIATED WITH
THE JCC, WITHOUT RESPECT TO LOCATION, THE UNDERSIGNED HEREBY AGREES TO THE
FOLLOWING:
<br><br>1. THE UNDERSIGNED
HEREBY RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE the JCC, its
directors, officers, employees, contractors, and agents (hereinafter referred
to as ?releasees?) from all liability to the undersigned, his personal
representatives, assigns, heirs, and next of kin for any loss or damage, and
any claim or demands therefore on account of injury to the person or property
or resulting in death of the undersigned, whether caused by the negligence of
the releasees or otherwise while the undersigned is in, upon, or about the
premises or any facilities or equipment therein, or participating in any
program affiliated with the JCC, without respect to location.
<br><br>2. THE UNDERSIGNED
HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasees and each of
them from any loss, liability, damage, or cost they may incur due to the
presence of the undersigned in, upon, or about the JCC premises or in any way
observing or using any facilities or equipment of the JCC or participating in
any program affiliated with the JCC whether caused by the negligence of the
releasees or otherwise.
<br><br>3. THE UNDERSIGNED HEREBY ASSUMES FULL
RESPONSIBILITY FOR AND RISK OF BODILY INJURY, DEATH, OR PROPERTY DAMAGE due to
negligence of releasees or otherwise while in, about, or upon the premises of
the JCC and/or while using the premises or any facilities or equipment thereon
or participating in any program affiliated with the JCC.
<br><br>THE UNDERSIGNED further expressly agrees that
the foregoing RELEASE, WAIVER AND INDEMNITY AGREEMENT is intended to be as
broad and inclusive as is permitted by the law of the state of Florida and that
if any portion thereof is held invalid, it is agreed that the balance shall,
notwithstanding, continue in full legal force and effect.
