Самый простой вариант если я Вас правильно понял:

if ($_POST['field1']) $field1 = $_POST['field1']; // если заполнено поле 1
if ($_POST['field2']) $field2 = $_POST['field2']; // ..поле 2
if ($_POST['field3']) $field3 = $_POST['field3']; // ..поле 3
...

$title = $field1 . $field2 . $field3; // формируем title
